At Osborne, our guiding principles reflect our commitment to change, shared humanity, and the pursuit of justice and equity. We are seeking a Case Manager for our Kinship Reentry Program. This role involves delivering comprehensive services to families, including referrals, counseling, and crisis intervention/prevention. The Case Manager will coordinate and implement services while monitoring and evaluating outcomes for each family. Maintaining strong relationships with referral partners and reentry service providers is essential, as is providing case management support to participating families. This position requires effective management of a caseload and will report directly to the Program Coordinator.

Compensation: $27.47 per hour

Key Responsibilities:

  • Conduct direct service activities, including assessments and intakes.
  • Document service interactions and case notes in Osborne's database.
  • Prepare and submit reports and correspondence to validate program processes as required.
  • Review screening forms for new participants and make necessary referrals.
  • Compose detailed progress notes that capture essential participant information.
  • Provide supportive counseling to families.
  • Engage in crisis intervention as necessary.
  • Conduct routine home visits.
  • Monitor and document client progress towards service plan objectives, ensuring all interactions are recorded.
  • Identify and access external services and resources as needed.
  • Facilitate orientation sessions.
  • Prepare for audits as required.
  • Follow up with participants to assess satisfaction and progress.
  • Contribute ideas for program development and innovation.
  • Attend required training sessions.
  • Support effective recruitment and outreach initiatives.
  • In-person collaboration with colleagues is mandatory.
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree in criminal justice, social work, or psychology; or a High School Diploma with 5 years of relevant case management experience.
  • Minimum of one year of case management or related experience.
  • Experience working with families and individuals involved in the criminal justice system.

Essential Skills:

  • Strong interpersonal skills and attention to detail.
  • Excellent verbal and written communication, time management, and organizational abilities.
  • Flexibility to work evenings and occasional weekends as required.
  • Proficiency in CARES, MS Word, Excel, Google Suite, and database management.
  • Familiarity with community resources.
  • Problem-solving, decision-making, and conflict resolution skills.
  • Ability to remain calm in crisis situations.
  • Local travel may be necessary.
  • Proven ability to collaborate effectively with diverse groups.
  • Capacity to manage multiple tasks efficiently under pressure.
