We are seeking a Patient Care Coordinator to join our team, who is passionate about delivering care in an empathetic and compassionate way. This role offers a unique opportunity to make a difference in the lives of patients suffering from end-stage renal disease or chronic kidney disease.
Estimated Salary: $40,000 - $60,000 per year
About the Role
The Patient Care Coordinator will work under the supervision of a Registered Nurse (RN) and will be responsible for providing direct patient care, including monitoring patients before, during, and after dialysis treatment. The successful candidate will have excellent communication skills, a strong attention to detail, and the ability to work effectively in a fast-paced environment.
Key Responsibilities
- Provide direct patient care, including measuring and recording vital signs, administering medications, and performing other tasks as assigned by the RN.
- Monitor patients during dialysis treatment, including responding to emergencies and providing support as needed.
- Work collaboratively with the interdisciplinary team to ensure high-quality patient care.
Requirements
- Certified Medical Assistant (CMA) certification or equivalent experience.
- High school diploma or equivalent required; associate's degree or higher preferred.
- Ability to work flexible hours, including evenings, weekends, and holidays.
- Strong communication and interpersonal skills.
