Job Summary
Hospice Savannah Inc. is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Care Coordinator. This role requires the ability to provide high-quality patient care, coordinate services, and maintain accurate records.

Responsibilities
- Provide direct patient care, including assessment, planning, and implementation of individualized care plans
- Coordinate services with healthcare providers, family members, and other stakeholders
- Maintain accurate and detailed records of patient care, treatment, and progress
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans

Requirements
- Graduate of an accredited school of nursing (BSN preferred)
- Current RN licensure in the state of Georgia
- Minimum of one year of experience in hospice, oncology, med-surg, or home health
- Certification in hospice or oncology preferred
- Excellent communication and interpersonal skills

Work Environment
This position requires moderate visual concentration for patient charting/documentation. The RN may be required to remain on site at Hospice House during meal breaks and perform patient visits utilizing their personal vehicle. Extensive driving may be required, exceeding 500 miles per week.
