The Role

We are seeking a Compassionate Service Coordinator to join our interdisciplinary team. As a key member of our Assertive Community Treatment (ACT) team, you will provide flexible, community-based services to promote the stability, recovery, and reintegration of individuals with mental health challenges.

Key Responsibilities

  • Engage in relationship building/rapport building with clients and provide service with a client-centered, harm reduction, and strengths-based approach.
  • Provide one-to-one support via regular home visits, focusing on life skills, housing concerns, and health maintenance.
  • Assist clients in navigating the housing path, including viewing units, assistance with moving in, and life skill development.
  • Coordinate and monitor referrals to community services, advocate for client participation, and manage contacts with outside agencies.
  • Develop a working knowledge of relevant community resources and explore new or unfamiliar resources as necessary.

Requirements

  • Able to work within a Housing First, client-centered, harm reduction framework.
  • Patient, creative, flexible, compassionate, and sensitive to persons with disabilities and other marginalized populations.
  • Good understanding of issues facing persons with mental health challenges or substance use issues.
  • Excellent independent problem-solving skills and ability to approach challenging situations intuitively.
  • Valid driver's license, reliable personal vehicle available for work purposes, and current automobile insurance required.

Compensation

  • $20-$21/hour; starting salary with up to an 8.5% increase after the first year of employment.
  • $3000 hiring bonus available.
