Job Summary

Care Network is seeking a highly motivated and organized Admissions Coordinator to join our team. As an Admissions Coordinator, you will play a critical role in establishing referral sources and admitting residents in an efficient manner.

Key Responsibilities:
  • Coordinate and facilitate admissions to our facility, ensuring seamless transitions for patients and families.
  • Maintain contact with patients/families regarding potential admissions, providing exceptional customer service and support.
  • Conduct interviews and tours of the facility, showcasing our services and amenities.
  • Verify insurance coverage and benefits, ensuring accurate and timely admission processes.
  • Educate family members, clients, and prospective accounts regarding our services and programs, highlighting our unique value proposition.
  • Establish relationships with social workers, case managers, and other professional contacts within hospital accounts or assigned accounts, generating new referrals and growing our network.
  • Assist with the development and maintenance of physician relations, as assigned, fostering strong partnerships and expanding our reach.
  • Complete weekly, monthly, and other reports, as assigned, in a timely and comprehensive manner, providing valuable insights and metrics.
  • Maintain current knowledge and understanding of client payment sources, including HMO, commercial insurance, Medicare, Medicaid, and other relevant programs.
  • Compile weekly referral log summaries, monthly nursing summaries, and monthly referring facility summaries, among other required reports, ensuring accuracy and attention to detail.
Requirements:
  • A self-starter with excellent communication and interpersonal skills, able to build rapport with diverse stakeholders.
  • Strong organizational and prioritization skills, with the ability to manage multiple tasks and deadlines effectively.
  • Analytical and problem-solving skills, with the capacity to think critically and creatively.
  • Proficiency in Microsoft Office and other relevant software applications.
