About the Role

We are seeking a Support Services Coordinator - Community Living to join our team. As a vital member of our Specialized Services Program, you will provide individualized supports to individuals with intellectual disabilities or Autism Spectrum Disorder residing in the community.

Job Description

Your primary responsibilities will include:

  • Facilitating conversation and activities of interest to the individual, providing guidance, companionship, emotional support, and supervision for safety;
  • Advocating on behalf of consumers with family, peers, and community members as needed;
  • Introducing consumers to community resources and facilitating usage;
  • Encouraging and supporting participation in community events;
  • Taught Consumers safety skills in all aspects of community and home life;
  • Teach, instruct, and role-model appropriate community and social behavior and expectations as needed;
  • Monitoring emotional and physical well-being; administering medications as appropriate; educating and assisting with personal care as required;
  • Attending medical appointments and ensuring appropriate follow-up is occurring;
  • Providing transportation to desired activities, community events, and appointments;
  • Collaborating with clinical staff to ensure mental health needs are being met;

Required Skills and Qualifications

To be successful in this role, you will need:

  • A High School Diploma or Equivalent;
  • Relevant experience in the human services field, preferred;
  • A Valid Driver's License;
