Job Summary

The Patient Services Coordinator - LPN is a vital member of the HCR Home Care team, responsible for ensuring seamless patient care and scheduling operations. This role requires a skilled and detail-oriented individual to coordinate patient visits, manage scheduling, and maintain accurate records.

Key Responsibilities
  • Patient Scheduling: Coordinate patient visits, schedule appointments, and ensure timely completion of tasks.
  • Scheduling Management: Manage scheduling requests, update patient records, and maintain accurate scheduling information.
  • Communication: Communicate effectively with patients, clinicians, and other stakeholders to ensure smooth operations.
  • Record Keeping: Maintain accurate and up-to-date patient records, including scheduling information and task completion.
  • Problem-Solving: Identify and resolve scheduling conflicts, patient concerns, and other operational issues.
Requirements
  • Education: Graduation from an accredited program of practical nursing education and unexpired/unencumbered NYS LPN licensure.
  • Skills: Excellent communication and organizational skills, ability to work in a fast-paced environment, and proficiency in computer software and keyboarding.
  • Experience: Previous experience in a healthcare setting, preferably in a scheduling or coordination role.
Work Environment

The Patient Services Coordinator - LPN will work in a dynamic and fast-paced office environment, with occasional exposure to outdoor conditions.

Physical Requirements

  • Standing: Regularly stand for extended periods.
  • Sitting: Spend most of the day sitting.
  • Hearing: Regularly hear and respond to patient and clinician communications.
  • Walking: Occasionally walk to other areas of the office or to meet with patients.
  • Talking: Regularly communicate with patients, clinicians, and other stakeholders.
  • Stoop or Kneel: Occasionally stoop or kneel to access files or equipment.
  • Repetitive Motion: Regularly perform repetitive tasks, such as data entry and scheduling.
