Job Title: Patient Services Coordinator LPN

Job Summary: We are seeking a highly skilled Patient Services Coordinator LPN to join our team at HCR Home Care. As a Patient Services Coordinator LPN, you will play a critical role in ensuring the smooth operation of our clinical services.

Key Responsibilities:
  • Coordinate patient scheduling and care plans with healthcare providers and staff.
  • Manage and maintain accurate records of patient information, including medical history, treatment plans, and progress notes.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
  • Collaborate with interdisciplinary teams to develop and implement patient care plans.
  • Monitor and report on patient outcomes, identifying areas for improvement and implementing changes as needed.
  • Ensure compliance with regulatory requirements, policies, and procedures.
  • Participate in quality improvement initiatives and contribute to the development of best practices.
Requirements:
  • Graduation from an accredited LPN program and current licensure in New York State.
  • Minimum 1 year of experience in a healthcare setting, preferably in home health care.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong organizational and time management skills.
  • Proficiency in electronic medical records and other software applications.
Work Environment:

The Patient Services Coordinator LPN will work in a fast-paced office environment, with occasional travel to patient homes or healthcare facilities.

Physical Requirements: The position requires sitting for extended periods, with occasional standing, walking, and lifting of up to 20 pounds.
