Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at the Austin State Hospital. As a RN Case Manager, you will be responsible for providing comprehensive nursing care to individuals with intellectual and developmental disabilities who are medically fragile or have behavioral issues.

Key Responsibilities
  • Conduct thorough assessments of individuals' health status and develop individualized care plans
  • Provide hands-on nursing care, including guidance for LVNs and direct care staff
  • Collaborate with the interdisciplinary team to ensure consistent, high-quality care
  • Monitor and evaluate the effectiveness of care plans and make adjustments as needed
  • Participate in quality assurance reviews and contribute to the development of nursing policies and procedures
Requirements
  • Licensed to practice as a registered nurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact
  • At least one year of experience as a practicing registered nurse, with experience in assessment and documentation of health status
  • Preferred: two years of experience as a practicing registered nurse in a health agency, hospital, or healthcare facility; Bachelor's Degree; previous experience working with individuals with intellectual and/or developmental disabilities
