The RN Case Manager engages in advanced nursing practices, focusing on the assessment, care, and integrated treatment of individuals residing at our facilities.

This role involves overseeing the necessary nursing care and monitoring the health status of individuals within your caseload.

As a vital member of the interdisciplinary team (IDT), you will collaborate closely with other professionals to ensure effective communication and the delivery of high-quality, holistic care.

Your responsibilities will include conducting physical assessments, documenting healthcare needs, developing care plans, and evaluating the outcomes of nursing interventions.

Proactively working to achieve optimal health outcomes for assigned individuals is essential. You will also review documentation for any issues related to your caseload and develop comprehensive nursing care plans.

Continuous professional development is encouraged, and you will utilize established nursing methods and procedures to fulfill your duties effectively. Participation in medical appointments with individuals will be necessary to ensure seamless communication and continuity of care.

Engagement in quality assurance reviews, mentoring new RN case managers, and participating in various committees and workgroups will also be part of your role.

Under the supervision of a Registered Nurse manager, you will have the autonomy to exercise initiative and independent judgment.


Key Responsibilities:


Regular attendance is required, and you may need to work specific shifts, including rotating schedules and overtime as per agency policies.

You may also serve on an on-call basis as needed to meet operational demands.


Monitor the health status of all individuals in your care, conducting rounds to gather information from direct support professionals and nursing staff. Your observations and interactions will be crucial in providing effective nursing interventions.

Utilize your nursing skills and established protocols to identify and address current and emerging health issues. Documentation and follow-up on these matters will be essential.

Ensure proactive nursing care is delivered to promote the best health outcomes for those served.

Demonstrate a thorough understanding of nursing processes and procedures, providing hands-on care as necessary, and guiding LVNs and direct care staff in implementing health care plans.


Actively participate as a nursing expert within the interdisciplinary team, contributing to problem-solving related to care and nursing processes.

Educate team members, family representatives, and other medical providers about the health care plans and status of individuals.

Work towards consensus within the interdisciplinary team and advocate for the needs and preferences of individuals.


Prepare for and participate in essential meetings related to your caseload, ensuring effective management and coordination of health care.

Update necessary records in automated systems and communicate relevant information to other disciplines.


Conduct physical assessments regularly and as needed, using the information to develop acute care plans and referrals to healthcare providers.

Complete required documentation consistently and in accordance with established guidelines.


Review physician orders and medication records to ensure proper administration of treatments.

Attend medical appointments with individuals and coordinate continuity of care as required.


Engage in ongoing training and education to maintain nursing best practices and ensure a safe working environment.

Establish and maintain professional relationships with individuals, colleagues, and visitors, while mentoring new RN Case Managers.


Participate in quality assurance activities to enhance nursing policies and procedures, contributing to the overall improvement of care provided.

Other duties may include supporting agency obligations for disaster response and continuity of operations.


Knowledge, Skills, and Abilities:

  • Understanding of nursing techniques, standards, and regulations.

Licensure Requirements:
Must be licensed to practice as a registered nurse in Texas or a state that recognizes reciprocity through the Nurse Licensure Compact.

Initial Selection Criteria:
Minimum of one year of experience as a practicing registered nurse is required, with preference given to those with experience in health agencies or facilities.

A Bachelor's Degree is preferred, along with experience working with individuals with intellectual and developmental disabilities.
