We are seeking a highly skilled Registered Nurse Case Manager to join our team at Heart Hospital of Austin. As a key member of our healthcare team, you will be responsible for ensuring high-quality, patient-centered care by managing rehabilitative care to achieve optimum outcomes.
Key Responsibilities- Provide program orientation to patients, families, and caregivers, including the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
- Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
- Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
- Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
- Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.
- 1+ years' experience in case management.
- Associate Degree in Nursing or Nursing Diploma.
- Bachelor's Degree in Nursing – preferred.
- Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation.
- Certification in Case Management – preferred.
Heart Hospital of Austin is a 58-bed hospital specializing in the diagnosis and treatment of cardiovascular disease. We feature a comprehensive 24-hour emergency department and an advanced Executive Wellness Program. Our six operating rooms, including three hybrid ORs, serve the city of Austin.
