Job Summary

We are seeking a skilled Registered Nurse to join our Case Management team at Ascension Seton Northwest. As a Nurse Case Manager, you will play a critical role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Requirements
  • Licensed Registered Nurse credentialed from the Texas Board of Nursing
