About the Role

We are seeking a highly skilled Registered Nurse Case Manager to join our team at Heart Hospital of Austin. As a key member of our rehabilitation team, you will play a vital role in ensuring high-quality, patient-centered care for our patients.

Responsibilities

  • Provide program orientation to patients, families, and caregivers, including an overview of the case manager's role, rehabilitation philosophy, and continued stay and discharge criteria.
  • Act as the coordinator of patient, family, and caregiver education, promoting participation in team discussions related to plans, goals, and status.
  • Ensure the implementation of the patient's treatment plan, supporting the patient's strengths, abilities, needs, and preferences.
  • Document the findings of the Discharge Planning Evaluation (DPE) and psychosocial assessments, communicating social, financial, or discharge needs and preferences of the patient, family, or caregiver.
  • Assume accountability for promoting consistent, positive patient interactions that advance the agenda of unparalleled patient service.

Requirements

  • 1+ years' experience in case management
  • Associate Degree in Nursing or Nursing Diploma
  • Bachelor's Degree in Nursing - preferred
  • Currently licensed as a Registered Nurse in the state(s) of practice according to law and regulation
  • Certification in Case Management - preferred
