Job Summary

We are seeking a highly skilled Registered Nurse Case Manager to join our team at St. David's Medical Center. As a key member of our healthcare team, you will be responsible for coordinating patient care, managing length of stay, and ensuring appropriate resource utilization.

Key Responsibilities
  • Provide case management services for inpatient and observation patients, identifying those at risk for adverse outcomes during transitions of care.
  • Conduct comprehensive assessments of patients' psychosocial, medical, and discharge needs, as well as their access to resources.
  • Develop and implement discharge plans in collaboration with the multidisciplinary healthcare team, including the patient's physician.
  • Partner with Social Services to ensure post-acute medical needs and level of care are appropriate.
Requirements
  • 2+ years' experience in case management or 3+ years' experience in clinical nursing.
  • InterQual experience preferred.
