Key Responsibilities- Conduct comprehensive assessments and develop individualized care plans to meet the unique needs of each person served.
- Collaborate with healthcare providers, family members, and other stakeholders to ensure seamless care coordination and communication.
- Monitor health status, identify trends, and implement interventions to promote optimal health outcomes.
- Participate in quality assurance reviews, attend meetings, and contribute to the development of policies and procedures.
- Provide education and support to individuals, families, and caregivers to promote self-advocacy and empowerment.
- Current licensure as a Registered Nurse in the state of Texas.
- Minimum of 2 years of experience in case management or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Strong analytical and problem-solving skills.
