Job Overview:

As a vital member of our organization, the Case Manager plays a crucial role in supporting clients through comprehensive assessments and tailored service plans.

Key Responsibilities:

1. Conduct thorough evaluations of client needs and document findings effectively.

2. Collaborate in creating personalized service plans that align with client assessments.

3. Facilitate the provision of essential services identified during assessments.

4. Assist clients in navigating access to necessary services through referrals and connections.

5. Monitor client progress by tracking appointments, following up on services, and reassessing needs periodically.

6. Advocate for clients by engaging in activities such as writing letters to expedite services and empowering clients and families to advocate for themselves.

7. Maintain accurate case records that document interactions, required services, progress reports, and other relevant information, adhering to organizational and regulatory standards.

8. Provide case consultation by collaborating with service providers to assess client status and progress.

9. Communicate effectively with supervisors regarding case management issues, resource needs, and service collaboration.

10. Deliver psychosocial education to clients and their families to enhance understanding of their situations.

11. Offer crisis intervention services, including emergency referrals and coordination of urgent support.

12. May assume the role of Primary Case Coordinator, with responsibilities varying based on experience and autonomy.

This job description aims to outline the primary duties and responsibilities of the Case Manager role. Employees may be required to perform additional job-related tasks as needed.

Qualifications:

• A Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field recognized by Medicaid or applicable funding sources.

• Completion of a state-mandated case management/service coordination course within six months of employment.

Experience:

ADULT DIVISION: A minimum of one (1) year of full-time experience working directly with adults or the ACT population in a human services environment is required post-Bachelor's degree; relevant Master's degree holders may qualify without prior experience.
