Job Description:

As a Senior Case Manager/Service Coordinator at Seven Counties Services, you will play a vital role in providing case management or service coordination services to clients, promoting their independence in performing activities of daily living, and empowering them to manage their symptoms. Under routine-to-moderate supervision, you will work independently, in accordance with instructions, policies, and procedures, and accepted practices, but consulting regularly with your supervisor on particularly complex or otherwise challenging situations.

Key Responsibilities:
  1. Conduct comprehensive assessments of clients and/or their needs, documenting the results.
  2. Participate in the development of client service plans, consistent with assessments.
  3. Arrange for the delivery of needed services, as identified in assessments.
  4. Assist clients in accessing needed services through referrals or linkages.
  5. Monitor client progress by making referrals, tracking appointments, performing follow-ups, and conducting periodic reassessments of changing needs.
  6. Perform advocacy activities on behalf of clients, such as writing letters to expedite entitlement payments or encouraging clients or family members to advocate on their own behalf.
  7. Prepare and maintain case records, documenting contacts, services needed, reports, client progress, and other relevant information, in accordance with SCS and regulatory standards.
  8. Provide case consultation, consulting with service providers and collaterals to determine client status and progress.
  9. Communicate with supervisors about issues related to case management, resourcing, service collaboration, and development of new resources.
  10. Provide psychosocial education to clients and/or family members to increase their understanding of client illness, physical condition, or social situation.
  11. Perform crisis assistance, intervening on behalf of clients, making arrangements for emergency referrals, and coordinating other needed emergency services.
Requirements:
  • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field as approved by Medicaid or required by applicable funding source.
  • Completion of state-required case management/service coordination course within six months of employment.
  • At least one year of full-time employment experience post-Bachelor's working directly with individuals under 21 years in a human service setting, with relevant Master's degree, or no experience.
Physical Demands/Requirements:
  • Lifting of 20 pounds maximum, frequent lifting and/or carrying of items weighing up to 10 pounds, or significant periods of walking or standing.
  • Frequent exposure to threat of bodily harm from clients or families.
  • Normal tasks may involve exposure to blood, bodily fluids, or tissue.
  • May involve 24-hour on-call responsibilities.
  • Reliable transportation for frequent travel.
