Job Summary:

As a Care Manager/Service Coordinator at Seven Counties Services, you will play a vital role in supporting clients with their unique needs. You will also facilitate referrals, linkages, and advocacy activities to promote client well-being. This role requires a Bachelor's degree in a related field and at least one year of experience working with adults in a human service setting.

Responsibilities:

Conduct comprehensive assessments and document client needs
Develop and implement personalized service plans
Arrange for delivery of necessary services
Facilitate referrals and linkages to support client well-being
Perform advocacy activities on behalf of clients
Maintain accurate case records and communicate with supervisors as needed

Requirements:

Bachelor's degree in Social Work, Psychology, Sociology, or related field
At least one year of experience working with adults in a human service setting
