Job Summary:

We are seeking a skilled Case Manager/Service Coordinator to join our team at Seven Counties Services. As a key member of our team, you will provide case management or service coordination services, promoting clients' independence in performing activities of daily living, and empowering clients to manage their symptoms.

Key Responsibilities:

• Conducts and documents comprehensive assessments of clients and/or their needs.
• Participates in the development of clients' service plans, consistent with assessments.
• Arranges for the delivery of needed services, as identified in assessments.
• Assists clients in accessing needed services through referrals or linkages.
• Monitors clients' progress by making referrals, tracking clients' appointments, performing follow-up on services rendered, and performing periodic reassessments of clients' changing needs.
• Performs advocacy activities on behalf of clients, such as writing letters to expedite entitlement payments or encouraging clients or family members to advocate on their own behalf.
• Prepares and maintains case records documenting contacts, services needed, reports, clients' progress, etc., in accordance with SCS and regulatory standards.
• Provides case consultation, consulting with service providers/collaterals in determining clients' status and progress.
• Communicates with supervisors about issues related to case management, resourcing, service collaboration, and development of new resources.
• Provides psychosocial education to clients and/or family members to increase their understanding of clients' illnesses, physical conditions, or social situations.
• Performs crisis assistance, intervening on behalf of clients, making arrangements for emergency referrals, and coordinating other needed emergency services.

Requirements:

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field as approved by Medicaid, or as required by applicable funding sources.
• Completion of state-required case management/service coordination course within six months of date of employment.
• At least one year of full-time employment experience post-Bachelor's working directly with individuals under the age of twenty-one (21) years in a human service setting, with relevant Master's degree, no experience.

Physical Demands/Requirements:

• Position requires lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing.
• Frequent exposure to threat of bodily harm from clients or families.
• Normal tasks may involve exposure to blood, bodily fluids, or tissue.
• May involve 24-hour on-call responsibilities.
• Reliable transportation for frequent travel.
