Job Summary

We are seeking a skilled Case Manager/Service Coordinator to join our team at Seven Counties Services. As a key member of our team, you will provide case management and service coordination services to clients, promoting their independence and empowering them to manage their symptoms.

Key Responsibilities
  • Conduct comprehensive assessments of clients and their needs, documenting findings and developing service plans.
  • Arrange for delivery of needed services, including referrals and linkages to community resources.
  • Monitor client progress, making referrals, tracking appointments, and performing follow-up on services rendered.
  • Perform advocacy activities on behalf of clients, including writing letters and encouraging clients or family members to advocate on their own behalf.
  • Prepare and maintain case records, documenting contacts, services needed, reports, and client progress.
  • Provide case consultation and communicate with supervisors about issues related to case management, resourcing, and service collaboration.
  • Provide psychosocial education to clients and/or family members to increase their understanding of client's illness, physical condition, or social situation.
  • Perform crisis assistance, including intervention on behalf of clients and making arrangements for emergency referrals and services.
Requirements
  • Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field as approved by Medicaid or required by applicable funding source.
  • Completion of state-required case management/service coordination course within six months of date of employment.
  • At least one year of full-time employment experience post-Bachelor's working directly with individuals under the age of 21 in a human service setting, with relevant Master's degree, no experience.
Physical Demands/Requirements
  • Lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds.
  • Frequent exposure to threat of bodily harm from clients or families.
  • Normal tasks may involve exposure to blood, bodily fluids, or tissue.
  • May involve 24-hour on-call responsibilities.
