Job Title: Service Coordinator II

Job Summary:

The Service Coordinator II is a critical role within our organization, responsible for planning, scheduling, and coordinating services for participants. This position requires a high level of expertise in human services, with a focus on supporting individuals with developmental disabilities.

Key Responsibilities:

  • Develop and implement Individualized Support Agreements for program participants.
  • Coordinate and facilitate services in accordance with participant Support Agreements and ISP.
  • Monitor program goals and objectives to ensure alignment with agency mission and strategic plan.
  • Provide counseling, guidance, and education to program participants as needed.
  • Liaise with state agencies, family members, and other involved parties to ensure quality care.
  • Complete performance reviews for employees and provide coaching and counseling as needed.
  • Develop and maintain a staff schedule to ensure sufficient qualified staff are available to meet programmatic needs.
  • Assist participants to navigate the Seven Hills Rhode Island system and other service delivery systems.
  • Report urgent and important issues regarding service delivery to supervisor.

Qualifications:

Requires a degree in Human Services or a related field, three years of progressive experience in human services with at least one year of management experience, or an equivalent combination of education and experience. Must possess computer skills and the ability to navigate a network system.
