HEALTHCARE PATIENT COORDINATOR JOB SUMMARY


The Healthcare Patient Coordinator plays a vital role in assisting patients, fostering trust, and guiding them as they learn to navigate their health information through digital platforms.

This position involves directing patients to essential resources and managing their information effectively.

HEALTHCARE PATIENT COORDINATOR ESSENTIAL DUTIES AND RESPONSIBILITIES
Patient Portal Management

Support patients in accessing their health information electronically and on demand.

Generate daily reports utilizing computer systems to monitor patient assistance activities.

Guide patients through the initial login process and provide an overview of portal functionalities.

Educate patients on how to communicate with clinics via messaging.

Assist patients over the phone who may encounter difficulties navigating the portal.

Deactivate portal accounts as necessary (e.g., when a patient turns 12 years old).

New Patient Engagement

Conduct phone calls following a structured protocol to all new patients to cover the following:

Welcome the patient and confirm their appointment.

Provide clear directions and parking information.

Address insurance inquiries.

Instruct patients to bring necessary documents to their first visit.

Welcome new patients and present educational materials, introducing all services available.

Schedule initial dental or optometry visits for patients who have not previously received these services (based on insurance coverage or out-of-pocket payment).

Assist patients in completing registration forms and enrollment in relevant programs.

Gather information on referral sources to enhance community outreach and marketing strategies.

Follow up with patients post-visit to assess satisfaction and remind them of upcoming appointments. Document any complaints and identify trends to recommend improvements.

Ongoing Patient Coordination

Collect Patient Satisfaction Surveys to evaluate service quality.

Ensure that waiting areas, triage, examination rooms, and staff workspaces are stocked with appropriate health education and marketing materials.

Outreach Activities

Contact and send welcome packets to new patients assigned to our healthcare providers.

Process applications for patient assistance programs.

Requirements

HEALTHCARE PATIENT COORDINATOR EDUCATION AND EXPERIENCE
High school diploma or equivalent; Associate Degree preferred.

Proficient in computer skills, including Microsoft Office and online navigation.

Experience with HIPAA regulations is preferred.

A minimum of 2 years of customer service experience, preferably in a healthcare setting.

Bilingual in Spanish is required.

$17- $19 hourly
