HEALTHCARE PATIENT COORDINATOR SUMMARY

Healthcare Patient Coordinators play a vital role in assisting individuals with their healthcare journey, fostering trust, and guiding them in managing their health information effectively. This position involves directing patients to essential resources and managing relevant information.

KEY RESPONSIBILITIES OF A HEALTHCARE PATIENT COORDINATOR

Patient Portal Management

  • Support patients in accessing their health information electronically and on demand.
  • Generate daily reports to monitor patient interactions and assistance.
  • Guide patients through the initial login process and provide an overview of portal functionalities.
  • Educate patients on how to communicate with clinics via the portal.
  • Assist patients over the phone with navigation issues related to the portal.
  • Manage portal account deactivation for patients upon reaching the age of 12.

New Patient Engagement

  • Conduct phone outreach to new patients following a structured protocol to cover:
  1. Welcoming the patient and confirming their appointment.
  2. Providing necessary directions and parking information.
  3. Addressing insurance-related inquiries.
  4. Instructing patients on required paperwork for their first visit.
  • Welcome new patients in person and provide educational materials about available services.
  • Schedule initial dental or optometry visits for patients who have not previously received these services.
  • Assist patients in completing registration forms and enrollment processes.
  • Gather information on how patients learned about the organization to enhance outreach efforts.
  • Follow up with patients post-visit to assess satisfaction and remind them of upcoming appointments.

Ongoing Patient Coordination

  • Collect feedback through Patient Satisfaction Surveys.
  • Ensure that waiting areas and exam rooms are equipped with appropriate health education materials.

Community Outreach

  • Reach out to new HMO patients assigned to our primary care providers.
  • Process applications for Patient Incentive Programs.

EDUCATION AND EXPERIENCE REQUIREMENTS

  • High school diploma or equivalent; an Associate Degree is preferred.
  • Proficient in Microsoft Office and online navigation skills.
  • Prior experience with HIPAA regulations is advantageous.
  • A minimum of 2 years of customer service experience, preferably in a healthcare setting.
  • Bilingual in Spanish is required.
