Responsibilities:
Assist and provide referral support to community members in navigating follow-up diagnostic evaluations for patients.

Establish and maintain effective communication between patients, their families, healthcare providers, and the health care system while offering additional support as necessary.

Enhance health care outcomes and mitigate health disparities for low-income, uninsured adult patients.

Conduct face-to-face visits with patients to perform assessments and develop care plans.

Monitor patients throughout their treatment and refer them to community-based organizations to address care gaps.

Provide post-treatment follow-up, contact patients, and update care plans as required.

Integrate prevention and screening recall systems into electronic medical records.

Assist in data collection, entry, evaluation, and program reporting.

Support supervisors in compiling, analyzing, and summarizing data and information for program reporting requirements.

Participate in meetings as necessary.

Qualifications:
Minimum Qualifications

Associate's degree in a related field.

Bilingual in English and Spanish.

A minimum of three years of relevant experience within the healthcare system, clinic environment, or other community-based organizations. Equivalent related experience may substitute for the degree.

Preferred Qualifications

Bachelor's degree in a related field with two years of experience in care coordination or health education.

Required Skills/Knowledge/Abilities

Ability to work flexible hours.

Proficient in presenting information effectively both verbally and in writing to individuals and groups from diverse cultural, ethnic, and educational backgrounds.

Experience with computers, Microsoft Office products (Word/Excel), business email communication systems, and internet search capabilities.
