Medication Assisted Treatment Nurse Care Manager

We are seeking a highly skilled and compassionate Nurse Care Manager to join our team at Sea Mar Community Health Centers. As a key member of our Medication Assisted Treatment (MAT) program, you will play a vital role in providing comprehensive care to individuals struggling with substance use disorder.
Responsibilities- Provide individualized care to patients, including intake assessments, program orientation, and ongoing treatment and follow-up.
- Collaborate with MAT providers and other healthcare professionals to ensure seamless care coordination.
- Develop and implement individualized care plans that address the unique needs of each patient.
- Monitor patient progress and adjust treatment plans as needed.
- Ensure accurate and timely documentation of patient services and progress.
- Current licensure as a Registered Nurse in the State of Washington.
- Experience working with individuals with substance use disorder and their families.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Basic knowledge of harm reduction approaches and trauma-informed care.
