Care Navigator

Evergreen Treatment Services is seeking a highly motivated and compassionate Care Navigator to join our REACH team. As a Care Navigator, you will play a critical role in helping individuals experiencing homelessness and behavioral health challenges achieve stability and improved quality of life.
Key Responsibilities- Provide care coordination, outreach, engagement, and case management services to adults experiencing behavioral health challenges, homelessness, and substance use disorders.
- Link individuals with Opioid Use Disorder to Medication for Opioid Use Disorder (MOUD) care and harm reduction services.
- Conduct comprehensive assessments to understand clients' substance use patterns, mental health status, and social determinants of health.
- Develop individualized harm reduction plans in collaboration with clients and care team.
- Provide linkage, referrals, and warm handoffs to needed services.
- Assist patients in overcoming barriers to engaging in MOUD care.
- Provide support to facilitate ongoing patient engagement with their MOUD service providers.
- High School Diploma or GED required; additional education preferred.
- Minimum of three years of experience in social work, outreach programs, or a related field.
- Knowledge of the medications used to treat opioid use disorder and ability to offer options counseling.
- Experience providing services from a harm reduction perspective is highly desirable.
- Competitive salary range: $79K to $81K.
