Position Title: MAT Nurse Care Manager

Compensation: Hourly Rate: $35 - $30
Annual Salary: $75,000 - $85,000

Role Summary:

The MAT Nurse Care Manager position is a full-time role based at our Seattle Medical Clinic, requiring travel to other Sea Mar medical facilities in the region. This position is designed to engage patients across multiple locations, providing enrollment services, monitoring treatment progress, and collaborating with the MAT Provider at local clinics. The role emphasizes individualized patient care aimed at minimizing harm to both patients and the community, supporting the MAT Program's objective of facilitating comprehensive recovery from substance use disorders.

Key Responsibilities:

  • Exhibit a comprehensive understanding of addiction as a chronic condition and implement evidence-based interventions, including Medication Assisted Treatment for individuals with substance use disorders.
  • Engage actively with individuals using illicit substances through a collaborative, person-centered approach that emphasizes harm reduction and trauma-informed care.
  • Deliver continuous addiction treatment and follow-up for patients, encompassing intake assessments, program orientation, stabilization, recovery maintenance, and relapse prevention.
  • Demonstrate the necessary knowledge and skills to provide appropriate care, functioning as a consultation nurse and collaborating closely with MAT providers based on patient needs.
  • Work as part of a team, coordinating care with other MAT Program staff and MAT Providers, and collaborating with Sea Mar administration to secure necessary resources for patient success.
  • Ensure adherence to Sea Mar policies, documenting any deviations and reporting findings to the Program Director promptly.
  • Facilitate communication between the MAT provider and pharmacy regarding prescriptions and refills for MAT-related medications, assisting with processing and addressing any concerns.
  • Conduct patient callbacks, medication counts, and observed urine drug screenings as part of the diversion control plan.
  • Maintain accurate documentation of services in the electronic health record within 24 hours, reflecting patient progress and aligning with care plans.
  • Collaborate with MAT Providers and Medical Clinic staff to support MAT Clinics in the assigned region.
  • Engage in quality assurance activities, including data collection for patient tracking and outcome measures required by grants.
  • Ensure compliance with all medical, nursing, and MAT policies, reporting deviations to the Program Director immediately.
  • Order, track, store, and administer injectable medications in accordance with Sea Mar policy, documenting any deviations.
  • Perform additional duties as assigned by MAT Program Leadership within the scope of employment and licensure.

Qualifications:

The ideal candidate must possess strong problem-solving abilities and the capacity to navigate various situations with limited standardization. Independent decision-making skills and a keen sense of medical priorities are essential. The candidate should be adept at anticipating the needs of patients and providers based on clinical presentations and historical trends.

Education and Credentials:

The Nurse Care Manager must be a Registered Nurse licensed in the State of Washington, having graduated from an accredited institution with an Associate's degree or higher. Experience in patient engagement aligned with Harm Reduction approaches and working with individuals with substance use disorders is preferred.

Training Requirements:

Completion of an initial Training Plan within the first two weeks of employment is mandatory, covering position-specific tasks, program philosophy, and essential knowledge for employment at Sea Mar. Basic overdose education and Naloxone training are also required.

Additional Requirements:

  • Pre-hire and annual employee health screenings are required.
  • Annual influenza vaccination is mandatory, with exceptions for approved medical or religious exemptions.
  • Annual TB testing is required.

Conditions of Employment:

The Nurse Care Manager must sign a confidentiality oath regarding patient information and undergo a background check. Compliance with health screening requirements for tuberculosis and immunization status is also necessary.

Language Skills:

Proficiency in reading and writing in English is required. The ability to speak and interpret in Spanish is preferred. Clear and concise communication skills, both written and verbal, are essential.

Mathematical Skills:

The candidate must be capable of calculating medical dosages and analyzing simple data for reporting purposes.

Reasoning Abilities:

The Nurse Care Manager should possess strong reasoning skills to solve practical problems and make independent decisions based on medical priorities and patient needs.

Work Environment:

The work environment is typically moderate in noise level, and the Nurse Care Manager may share workspace with other staff members. Interaction with clients exhibiting challenging behaviors associated with substance use disorders is expected.

Physical Requirements:

The role requires regular walking, talking, and hearing, with frequent standing and use of hands. The employee must be able to lift and move up to 10 pounds regularly and up to 50 pounds occasionally, with potential exposure to communicable diseases.
