POSITION OVERVIEW


The Nutrition Services Coordinator plays a vital role in assisting the Medically Tailored Meals team by providing expertise in nutritional planning, program enhancement, and quality control.


KEY RESPONSIBILITIES
1. Assess eligibility and intake for Medically Tailored Meals services.

2. Perform comprehensive nutrition evaluations.

3. Create and execute personalized nutrition strategies.

4. Review and oversee all meal and grocery plans associated with Medically Tailored Meals.

5. Ensure that menu selections are nutritious and culturally sensitive.

6. Organize and lead regular nutrition workshops and training sessions.

7. Collaborate with other agencies and providers to maintain seamless care.

8. Manage medical referrals in conjunction with contracted managed care organizations.

9. Participate in weekly case discussions.

10. Engage in program and organizational training as required.

11. Keep precise participant records in various information management systems and produce reports as needed.

12. Uphold confidentiality standards in accordance with HIPAA regulations.

13. Maintain a well-organized filing system.

14. Ensure that intake and care processes reflect harm reduction and trauma-informed care principles.

15. Regularly check and respond to communications.

16. Adhere to confidentiality protocols.

17. Perform additional duties as assigned.

QUALIFICATIONS AND SKILLS

• Proficient in spoken and written English at a professional level.

• High level of confidentiality is essential.

• Ability to support and advocate for The Salvation Army's mission.

• Proficient in computer applications, particularly within a Windows environment (Microsoft Office Suite).

• Exceptional communication skills, both verbal and written.

• Professional telephone etiquette and demeanor.

• Strong organizational capabilities.

• Excellent time management skills and ability to handle multiple responsibilities.

EDUCATIONAL REQUIREMENTS

• Bachelor’s degree in Dietetics, Food and Nutrition, or a related discipline.

• Credentialed as a Registered Dietitian by the Commission on Dietetic Registration (CDR) is mandatory.

• Food handler's certification is required.

• A valid California Class C Driver License is necessary, along with the ability to operate a Salvation Army vehicle.

• Candidates must be at least 21 years of age.

• Authorization for The Salvation Army to enroll in the CA DMV Pull Notice Program is required.

• Completion of The Salvation Army vehicle training course is necessary.

PHYSICAL DEMANDS:
• Capability to sit, walk, stand, bend, squat, climb, kneel, and twist intermittently or continuously.

• Ability to grasp, push, and pull objects.

• Ability to reach overhead.

• Capability to operate a telephone.

• Ability to lift objects weighing up to 25-40 lbs.

• Proficient in using a computer.

• Ability to process written, visual, and verbal information.

• Proficient in operating basic office equipment such as PCs, fax machines, telephones, calculators, copiers, and printers.

Qualified individuals must be able to perform the essential duties of the position with or without accommodation.

This position is funded by a grant and its continuation is contingent upon the renewal of the grant funding.

Employment in this role is dependent on the ongoing availability of funding, and there is no guarantee of continuation beyond the current funding period.
