POSITION SUMMARY
This role involves the planning, organization, and delivery of nutritional education for personnel within the organization. The individual will collaborate as part of the home care team, providing expert nutritional counseling.

KEY RESPONSIBILITIES
  • Designs and executes a tailored nutritional strategy based on a thorough assessment of dietary needs, integrating this plan into the overall patient care strategy.
  • Assesses and communicates the outcomes of nutritional interventions effectively.

Guides home care staff to:
  • Offer suitable counseling to patients and their families/caregivers in a home environment.
  • Assess and educate on nutritional guidelines, meal planning, food choices, and cost-effectiveness.
  • Modify dietary plans to align with the individual's lifestyle requirements.
  • Reassess nutritional care through ongoing home visits.

  • Documents relevant dietary history and nutritional care information in the clinical records.
  • Records subjective and objective data, assessments, and treatment plans in clinical notes, including discharge summaries.
  • Coordinates with clinicians regarding patient progress and treatment objectives.
  • Ensures the confidentiality and security of patient clinical records.
  • Adheres to the organization's policies and protocols.
  • Engages in continuing education programs.
  • Undertakes additional responsibilities as assigned.

QUALIFICATIONS
  • Preferably holds a Master's degree from an accredited institution in the fields of nutrition, dietetics, or food science, along with successful completion of an internship or equivalent experience.
  • Completed coursework in nutrition counseling methods and principles.
  • Licensed by relevant state authorities, if applicable.
  • Registered member of the American Dietetic Association.
  • Current CPR Certification is required.
  • Must possess a valid driver's license and have access to a vehicle that meets state or organizational insurance requirements and is in good operational condition.
