Job Summary

Zepf Center is seeking a highly skilled and compassionate Case Manager to join our team. As a key member of our behavioral health team, you will provide comprehensive support and coordination of services to individuals experiencing mental illness, ensuring their needs are addressed and goals are achieved.

Key Responsibilities
  • Assess client strengths and needs, developing a comprehensive care coordination and treatment plan in collaboration with the client and applicable team members.
  • Maintain regular contact with clients, providing support and advocacy to ensure stability and progress toward goal attainment.
  • Link and refer clients to other services, programs, and agencies as needed, ensuring access to entitlements and community supports.
  • Collaborate with supervisors and team members to provide the most effective and cost-efficient services, advocating for clients and communicating with family members and significant others as indicated.
  • Provide face-to-face contact with clients who are hospitalized, facilitating discharge planning and coordination of services.
  • Assist clients in tracking and keeping appointments, completing required paperwork in accordance with timelines and standards.
Requirements
  1. High School Diploma or GED required, with demonstrated mental health experience; or Bachelor's degree in social work or three years of mental health experience in behavioral health - preferred.
  2. CDCA certification required for SUD.
  3. Must have a valid driver's license and be able to provide proof of auto insurance.
Working Conditions

This is a fast-paced position that includes a high volume of individual and family contact, collaboration with various community agencies, as well as computer entry and tracking. The employee must be able to maintain a patient engagement standard and work in a culturally and racially diverse environment.
