About the Role

We are seeking a highly skilled and compassionate Ambulatory RN Care Coordinator to join our dynamic team at The University of Kansas Health System. As a key member of our rheumatology clinic, you will play a vital role in delivering exceptional patient care and ensuring seamless coordination of services.

Key Responsibilities
  • Manage Patient Flow and Triage: Coordinate patient arrivals, manage wait times, and ensure efficient use of clinic resources.
  • Communicate Effectively: Liaise with front desk staff, nursing staff, physicians, and clinic manager to ensure clear communication and coordination of patient care.
  • Develop Patient Outcomes: Collaborate with providers to set patient goals, define resource utilization, and work towards achieving optimal patient outcomes.
  • Provide Nursing Support: Offer clinical and educational programs to optimize patient outcomes and improve quality of life.
  • Coordinate Continuity of Care: Ensure seamless transitions of patient care between hospital admission, discharge, and ER visits.
  • Manage High-Risk Patient Care: Identify and manage patients with multiple comorbidities or high risk for readmission to a hospital setting.
Requirements
  • Education: Bachelor's degree in Nursing
  • Experience: 2+ years of RN experience
  • Licensure: Current active multi-state Registered Nurse (RN) license in good standing
Preferred Qualifications
  • Previous Medical Office Experience: Previous experience in a medical office setting
