Ambulatory Clinical Nurse Coordinator-cardiothoracic Job Summary

We are seeking a skilled Ambulatory Clinical Nurse Coordinator-cardiothoracic to join our team at The University of Kansas Health System. This role will provide exceptional patient care and coordination services to our cardiothoracic patients.

Key Responsibilities:
  • Manage patient flow, triage, and direct patient care in a fast-paced ambulatory setting.
  • Collaborate with front desk staff, nursing staff, physicians, and clinic manager to ensure seamless communication and care coordination.
  • Develop and implement patient care plans, setting realistic goals and expectations with patients and families.
  • Provide nursing support and coordination to optimize patient outcomes and improve quality of life through clinical and educational programs.
  • Support patient self-management of disease and coordinate continuity of care with patients and families.
  • Manage high-risk patient care, including patients with multiple comorbidities or high risk for readmission.
  • Develop relationships with patients and utilize assessment skills to identify physical, psychosocial, financial, and environmental healthcare needs.
  • Educate patients and ancillary personnel about healthcare needs, safety issues, and benefits.
  • Utilize electronic tools, such as EMR, to document assessments, clinical findings, plans of treatment, and progress toward goals.
Requirements:
  • High School Graduate
  • Bachelor Degree in Nursing
  • 2 or more years of RN experience
Preferred Qualifications:
  • Previous medical office experience
Knowledge Requirements:
  • Basic typing and word processing ability
  • Excellent analytical, critical thinking, and problem-solving skills
