Job Summary:

The Clinical Navigation Specialist at Kent Hospital plays a vital role in ensuring the delivery of quality and comprehensive services to oncology patients. This position is responsible for coordinating patient care throughout the continuum, collaborating with the multidisciplinary team, and serving as a clinical resource with expertise in oncology care management.

This role requires a strong understanding of oncology care processes and an ability to provide education and information to patients and their families. The successful candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.

Key Responsibilities:

  • Coordinate patient care throughout the continuum, ensuring seamless transitions between healthcare providers.
  • Provide education and information to patients and their families, helping them navigate the healthcare system and make informed decisions about their care.
  • Serve as a clinical resource for the multidisciplinary team, providing expert guidance on oncology care management.
  • Collaborate with healthcare providers to ensure comprehensive and coordinated care for oncology patients.
  • Maintain accurate and up-to-date documentation of patient interactions and care plans.

Requirements:

  • Bachelor of Science in Nursing required, with at least 5 years of nursing experience in Oncology.
  • Nurse Navigator experience preferred.
  • Current licensure as a Registered Nurse (RN) in Rhode Island and CPR certification.
  • Oncology Nursing Society certification required.
  • Nurse Navigation Certification Preferred and/or obtained within 6 months of being qualified.

Salary: $80,000 - $110,000 per year
