Healthcare Coordinator Lead

The Healthcare Coordinator Lead will oversee a team of healthcare coordinators and provide expertise in care coordination, case management, and disease education. This is a leadership opportunity for an experienced nurse looking to take their career to the next level.

About the Role:
  • Lead a team of healthcare coordinators to deliver high-quality care coordination services to patients with chronic health conditions.
  • Develop and implement care plans that address medical, social, and behavioral health needs.
  • Mentor and train staff on best practices in care coordination and case management.

Requirements:

  • Current licensure as a Registered Nurse (RN)
  • 5+ years of experience in healthcare, preferably in a leadership role
  • Strong leadership and communication skills

Salary: $74,000 per year, increasing with experience.
