Patient Care Coordinator

We are seeking a highly skilled Patient Care Coordinator to join our team at Kent Hospital. As a key member of our healthcare staff, you will play a vital role in providing exceptional patient care and support.
About the RoleThis is a fantastic opportunity for someone who is passionate about delivering high-quality patient care. The successful candidate will be responsible for performing various patient care activities, including assisting with personal hygiene, transcribing physician orders, and maintaining accurate medical records.
Key Responsibilities- Provide general patient care, assist with personal hygiene, and perform routine activities as directed by the Registered Nurse.
- Initiate, assemble, and maintain accurate functional charts for all patients in the assigned unit.
- Transcribe written physician orders, complete necessary forms, requisitions, and communications to implement orders and record clinical data.
- Maintain patient medical records in an orderly manner, document all care provided, and significant information, legibly, accurately, and completely.
The estimated annual salary for this position is $45,000 - $55,000, depending on experience.
