Job Description:

We are seeking a skilled Clinical Social Worker to join our team at Care New England Health System. As a key member of our healthcare team, you will be responsible for providing direct clinical social work services to patients and their families, collaborating with the interdisciplinary team, and developing effective care plans.

Key Responsibilities:
  • Conduct comprehensive psychosocial assessments and develop individualized care plans.
  • Collaborate with the healthcare team to coordinate care and ensure seamless transitions.
  • Provide education and support to patients and their families to promote healthy behaviors and coping strategies.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
Requirements:
  • A Master's degree in Social Work or a related field.
  • At least 3 years of experience in a healthcare setting, preferably in a hospital environment.
  • Strong leadership and communication skills.
Estimated Salary Range:

$80,000 - $100,000 per year, depending on experience.
