POSITION TITLE: Case Manager - Housing

FLSA CLASSIFICATION: Non-Exempt

REPORTS TO: Director of Residential Clinical Services and/or Director of Homeless Services

POSITION SUMMARY: The Housing Case Manager is responsible for delivering specialized case management and community psychiatric support for individuals in designated Residential Care Housing Programs. Utilizing a Housing First approach, the Case Manager collaborates with clients to create personalized care plans, coordinating services with treatment team members and community partners. The role emphasizes enhancing clients' independent living skills, fostering mental health recovery through person-centered care, and assisting in securing and maintaining stable housing. The Case Manager focuses on self-management of conditions, minimizing inpatient stays and emergency room visits, and ensuring smooth transitions between primary care and inpatient facilities to help clients achieve optimal functioning. A harm reduction and trauma-informed methodology will be applied to support mental health and sobriety.

KEY RESPONSIBILITIES:

  1. Deliver services to meet productivity standards, ensuring a minimum of 5 client contacts daily.
  2. Maintain precise and timely documentation of services, ensuring all progress notes are signed within 24 hours of service delivery.
  3. Develop treatment plans and assist clients in establishing personalized recovery objectives through a strengths assessment process, with regular updates as necessary.
  4. Monitor client status and service effectiveness, providing necessary support and adjustments, while documenting changes in progress notes.
  5. Engage with clients to provide coaching and instruction aimed at achieving identified goals and promoting recovery, reintegration, and community stability.
  6. Facilitate learning of Activities of Daily Living (ADL) to promote independence through coaching and skill-building in areas such as social skills and medication management.
  7. Advocate for clients by connecting them with necessary referral services to meet their identified goals.
  8. Assist clients in housing searches and link them to community resources to secure and sustain affordable permanent housing.
  9. Support clients with entitlement advocacy and application processes for benefits such as SSI/SSDI and food assistance programs.
  10. Help clients navigate crisis situations, manage anger, resolve landlord/tenant disputes, and develop crisis WRAP plans for symptom management and conflict resolution.
  11. Guide clients in budget development and management, educating them on the importance of financial planning.

ADDITIONAL REQUIREMENTS:

  1. Maintain an acceptable attendance record, participating in agency meetings and training as required.
  2. Meet deadlines and ensure accuracy in reports, paperwork, and electronic timesheets.
  3. Exhibit a suitable level of technical knowledge for the role, responding to emails and messages daily.
  4. Produce the necessary quantity of work to fulfill job requirements.
  5. Collaborate effectively with team members, maintaining open communication and treating clients with respect and warmth.
  6. Demonstrate professionalism in verbal and written interactions, employing effective listening skills and customer service.
  7. Adapt to new assignments and changes in procedures, taking initiative to address tasks.
  8. Complete assigned work accurately and on time without needing reminders.
  9. Ensure safe transportation of clients in the community as needed.
  10. Collaborate with partner agencies for mobile response and stabilization services.

QUALIFICATIONS: A bachelor's degree in a human services field or four years of equivalent experience in human services is required. Computer literacy is essential. Preferred qualifications include knowledge of community resources, housing alternatives, and vocational services; experience working with individuals facing serious mental health and substance use challenges; strong written and verbal communication skills; and the ability to build rapport with clients and colleagues. A valid driver's license and access to a personal vehicle are necessary.

PHYSICAL DEMANDS: This position requires driving for community mobility, typing/data entry, writing, lifting/carrying up to 30 pounds, and bending/stopping/climbing stairs. All duties are essential functions subject to reasonable accommodation.
