POSITION TITLE: Housing Case Manager – Veterans Shelter

REPORTS TO: Housing Case Management Team Lead

POSITION SUMMARY: The Housing Case Manager for the Veterans Shelter will implement the Housing First Model to assist veteran families experiencing homelessness or at risk of homelessness, aiming to ensure housing stability. This role involves creating personalized care plans and coordinating services with veterans, treatment team members, and community partners. The Case Manager will support veterans in enhancing their independent living skills, foster mental health recovery through a person-centered approach, and help secure and maintain sustainable housing. The focus will be on self-management of health conditions, minimizing hospital admissions, and improving care transitions to achieve optimal functioning. A harm reduction and trauma-informed methodology will be applied, emphasizing recovery-oriented tasks to aid veterans in their mental health and sobriety journeys.

KEY RESPONSIBILITIES:

  1. Deliver services to meet productivity benchmarks, ensuring a minimum of 5 documented veteran interactions daily.
  2. Maintain precise and timely documentation of service delivery, ensuring all progress notes are signed within 24 hours.
  3. Develop comprehensive case files that reflect individualized assessments, interventions, housing stability plans, and document progress towards goals.
  4. Facilitate treatment planning and assist veterans in establishing personalized recovery objectives through strengths assessments, updating these quarterly.
  5. Engage with veterans to provide coaching and instruction necessary for achieving identified goals related to recovery, reintegration, and community living.
  6. Conduct eligibility screenings for veteran households and manage referrals for those deemed ineligible.
  7. Support veterans in crisis situations, including anger management and developing crisis intervention plans.
  8. Complete required intake assessments for program enrollment, addressing veteran status, income, housing barriers, and health needs.
  9. Conduct both office and home visits to assist with housing stability planning and document linkages to community resources.
  10. Engage in landlord outreach to identify safe and affordable rental options, providing advocacy as needed.
  11. Assist veterans in budget creation to identify affordable housing options and assess financial assistance needs.
  12. Build rapport with veteran households, providing information and referrals, and participating in outreach events.
  13. Attend community meetings to foster collaborative relationships with partner agencies and veteran service organizations.
  14. Support veterans with entitlement advocacy and application processes for various assistance programs.
  15. Collaborate with veterans on self-sufficiency planning to ensure long-term housing stability and emotional well-being.
  16. Submit recertification requests for supervisory review within designated timelines.
  17. Participate in HMIS training and ensure accurate data entry into the system.
  18. Monitor the status of veterans and provide necessary support in collaboration with the VA.
  19. Conduct follow-up outreach post-shelter discharge to facilitate successful transitions to community services.

  9. Ensure safe transportation of veterans as required.
  10. Collaborate with partner agencies for mobile response and stabilization services.
  11. Achieve goals and objectives as outlined in performance reviews.

QUALIFICATIONS: The Housing Case Manager for the Veterans Shelter is expected to possess a bachelor's degree in a human services field or equivalent experience in the human services sector. A licensed Master's or Bachelor's degree is preferred. Proficiency in computer skills is required, along with knowledge of community resources and veteran services. Experience working with veterans in areas such as homelessness, mental health, and crisis intervention is essential. Strong written and verbal communication skills, along with the ability to build rapport with veterans and staff, are necessary. A valid driver's license and access to a personal vehicle are required.

PHYSICAL DEMANDS: The role involves driving for community mobility, typing and data entry, lifting up to 30 pounds, and performing tasks that may require bending or climbing stairs.
