REPORTS TO: Director of Residential Clinical Services and/or Director of Homeless Services
POSITION SUMMARY: The Housing Case Manager (HCM) is responsible for delivering focused case management and community psychiatric supportive treatment for individuals in designated Residential Care Housing Programs. Utilizing a Housing First approach, the HCM collaborates with clients to create personalized care plans, coordinating services with treatment team members and community partners. The HCM aids clients in enhancing their independent living skills, fostering mental health recovery through a person-centered perspective, and securing sustainable housing. Services prioritize self-management of conditions, aiming to reduce reliance on inpatient care and emergency services, while facilitating smooth transitions between healthcare providers and inpatient facilities to support clients in achieving optimal functioning. The HCM employs a harm reduction and trauma-informed strategy to assist clients in their recovery journey.
KEY RESPONSIBILITIES:
- Deliver services to meet the productivity standard of 5 client contacts daily.
- Maintain precise and timely documentation of services, ensuring all progress notes are signed within 24 hours of service delivery.
- Develop treatment plans and assist clients in establishing individualized recovery objectives through a strengths assessment process, with updates occurring quarterly or as necessary.
- Monitor client status and the effectiveness of ongoing services, providing necessary support and adjustments as needed.
- Engage with clients to coach and guide them towards achieving their identified goals, focusing on recovery, reintegration, and community stability.
- Facilitate learning of Activities of Daily Living (ADL) to promote independence, including assistance with social skills and medication management.
- Advocate for clients between them and other referral services to meet their identified goals.
- Support clients in housing searches and connect them with community resources to secure and maintain affordable permanent housing.
- Assist clients with entitlement advocacy and application processes for benefits such as SSI/SSDI and food assistance programs.
- Provide support during crisis situations, including anger management and landlord/tenant dispute resolution, while developing crisis WRAP plans for symptom management.
- Guide clients in budget development and management, emphasizing the importance of financial literacy.
ADDITIONAL REQUIREMENTS:
- Maintain an acceptable attendance record, participating in required agency meetings and training.
- Ensure accuracy and timeliness of various reports and documentation.
- Exhibit a proficient level of technical knowledge relevant to the position.
- Produce sufficient work volume to meet job expectations.
- Collaborate effectively with team members, maintaining open communication and treating clients with respect and dignity.
- Demonstrate professionalism in verbal and written communications, employing effective listening skills.
- Adapt to new assignments and changes in procedures, taking initiative to address tasks independently.
- Complete assigned work accurately and on time without needing reminders.
- Ensure safe transportation of clients as required.
- Collaborate with partner agencies for mobile response and stabilization services.
QUALIFICATIONS: The Housing Case Manager should possess a bachelor's degree in a human services field or equivalent experience in the human services sector. Proficiency in computer skills is essential. Preferred qualifications include knowledge of community resources, housing options, and vocational services; experience working with individuals facing serious mental health and substance use challenges; strong verbal and written communication skills; and the ability to build rapport with clients and colleagues. A valid driver's license and access to a personal vehicle are required.
