HIV/AIDS Case Management Specialist

POSITION SUMMARY
This role is a vital component of the medical case management team dedicated to supporting individuals affected by HIV/AIDS. The specialist collaborates closely with primary healthcare providers and various state and local organizations. This position facilitates connections for clients to essential services, which may include: Ryan White programs, housing assistance, dental care, transportation, and more. Additionally, the specialist provides HIV prevention education and conducts rapid testing and counseling services. Funding for this position is sourced from Ryan White program grants and the Medical Practice Association.
ESSENTIAL FUNCTIONS
Case Management
Client Enrollment into Services
- Offers clients foundational information regarding Ryan White programs (Parts B, C, and D), the Affordable Care Act, patient assistance initiatives, housing options, dental services, food banks, Medicaid, and Medicare benefits, along with other supportive resources.
- Assists clients in enrolling in the Affordable Care Act.
- Conducts comprehensive evaluations of social service needs, including financial resources, support programs, transportation, and nutritional assessments.
- Maintains precise and thorough records of all case management activities.
- Performs additional duties as assigned.
Referral Services
- Provides referrals based on client evaluations, covering financial resources, support programs, transportation, and nutritional assessments.
- Accesses pharmaceutical assistance programs and coordinates appropriate pharmaceutical care for patients.
- Coordinates services with local, state, and federal agencies.
- Performs additional duties as assigned.
Ryan White Part B Standards of Care
- Communicates eligibility status to clients and external providers promptly and accurately.
- Documents all client interactions in a timely and precise manner.
- Updates client eligibility at least annually or as necessary.
- Adheres to the Kansas Department of Health and Environment's case management standards of care.
- Participates in state and local meetings as required.
- Ensures compliance with HIPAA regulations regarding patient confidentiality.
- Performs additional duties as assigned.
HIV Counseling and Testing
- Provides clients with updated prevention education materials.
- Administers HIV testing and counseling as requested.
- Performs additional duties as assigned.
Collaborative Efforts
- Works in collaboration with other members of the Ryan White Program and external partners.
- Participates in community health fairs and other outreach events.
- Serves on committees related to the Ryan White Program, KU, MPA, Midtown Clinic, KDHE, or others, as required by the position or through volunteer initiatives. Such events may include World AIDS Day, patient holiday celebrations, community outreach activities, educational and training programs, or fundraising efforts.
Reliable Attendance and Punctuality
POSITION QUALIFICATIONS
Certificates & Licenses:
Certification as an HIV/AIDS Counselor and Educator will be required (training provided).
Education:
A Bachelor's degree in social work, counseling, psychology, or a related health field. Relevant experience may substitute for formal education.
Experience:
- One to two years of related experience (including internships and/or volunteer work).
- Experience working with multi-stressed or socioeconomically disadvantaged populations.
- Preferred experience with Electronic Health Records (EHR), specifically eClinicalWorks.
Proficiency in Microsoft Office is a plus.
