  • Act as a Single Point of Contact: Serve as the primary point of contact for patients, physicians, and care providers, facilitating communication and ensuring timely information exchange.
  • Transition Management: Develop and implement transition plans, communicating with patients, families, and healthcare teams to ensure smooth transitions.
  • Patient Education: Assess and document learning needs, developing individualized education plans to empower patients and families.
  • Nursing Care Planning: Establish and implement nursing care plans for assigned patient populations, ensuring comprehensive and effective care.
  • Discharge Planning: Collaborate with healthcare teams to manage patient discharge plans, optimizing communication and ensuring timely discharges.
  • Process Improvement: Assist in the development and implementation of process improvement activities to enhance clinical, financial, and satisfaction outcomes.
  • Utilization Management: Collect and analyze data to fulfill utilization and regulatory requirements, ensuring compliance and optimal resource allocation.
  • Case Escalation: Identify and communicate issues related to case escalation, ensuring timely resolution and minimizing delays.
  • Collaboration and Communication: Establish and maintain collaborative relationships with physicians, medical directors, nurses, and other healthcare team members, facilitating effective communication and resolution of issues.
  • Mentorship and Education: Mentor internal team members on case management and managed care concepts, promoting knowledge sharing and best practices.
  • Performance Monitoring: Actively supervise outcomes, participating in quality planning and ensuring alignment with key performance indicators.
Requirements
  • Education: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)
  • Experience: 5 years of clinical experience, with at least 1 year in a care coordination or case management role
  • Licensure: Active Minnesota Registered Nurse license
  • Certifications: Case Management Certification (CMC) or Basic Life Support (BLS)
