About the Role

We are seeking a highly skilled Licensed Practical Nurse to join our team at Maplewood at Stony Hill LLC. As a key member of our healthcare team, you will play a vital role in providing exceptional care and support to our residents.

Key Responsibilities
  • Medication Administration: Safely administer medications to residents as prescribed by physicians.
  • Direct Care and Service: Provide direct care and support to residents, including assistance with daily living activities and promoting independence.
  • Teaching and Wellness Counseling: Educate residents on healthy lifestyle choices and provide wellness counseling to promote overall well-being.
  • Health Promotion: Develop and implement health promotion programs to enhance the physical and emotional health of residents.
  • Participation in Assessments: Participate in assessments under the supervision of the Resident Services Director (RN) to ensure accurate and comprehensive care planning.
  • Support of Residents and Families: Provide emotional support and guidance to residents and their families, addressing their concerns and needs.
  • Oversight of Certified Nursing Assistants: Supervise and mentor Certified Nursing Assistants to ensure high-quality care and support.
  • Training and Auditing: Conduct regular training sessions and audits to ensure compliance with regulatory requirements and maintain high standards of care.
Requirements
  • Licensed Practical Nurse: Currently licensed to practice in the state.
  • Experience: Two years of full-time or full-time equivalent clinical experience.
  • Specialized Skills: Experience working with older adults, with a strong preference for those with dementia and psych experience.
  • Physical Demands: Ability to perform tasks that may be physically demanding, including pushing, bending, and lifting up to 50lbs.
  • Communication Skills: Excellent communication and interpersonal skills, with the ability to maintain positive working relationships with residents, families, peers, and other staff members.
