The Practical Nursing Specialist plays a vital role within a collaborative healthcare team. This position supports medical professionals in assessing client needs and facilitating comprehensive care. The LPN will also deliver medication education and administer injectable treatments as necessary.

Employment Type: Non-Exempt, Full-Time

Compensation: Starting at $27.00/hour

This role is scheduled for second shift and includes weekend hours.

QUALIFICATIONS

Completion of a nursing degree or relevant healthcare certification; training as a licensed practical nurse; familiarity with Medication-Assisted Treatment; capability to incorporate recovery principles into patient care.

Strong interpersonal skills for engaging with clients; ability to identify personalized recovery support options; proficiency in report writing; capacity to effectively present information and address inquiries; fluency in a second language is advantageous; adept with electronic medical records.

Physical Requirements: Prolonged periods of sitting at a desk, utilizing the phone, and working on a computer. Must be able to lift up to 20 pounds occasionally.

A valid driver's license is required.

KEY RESPONSIBILITIES

  • Collaborate with agency medical providers to evaluate clients' medical needs, including withdrawal symptoms and medication side effects.
  • Assist in the Suboxone induction process by monitoring vital signs and documenting all relevant information in the medical record, conducting toxicology screens, and gathering previous lab results and physical assessments.
  • Document ongoing client progress, particularly concerning medication side effects.
  • Support physicians in the submission, routing, and completion of ordered lab work.
  • Provide in-person clinical assessments and medication education to clients and their families.
  • Exhibit competency in addressing age-related or specialty issues and developmental needs for each population served.
  • Coordinate with prescribers and families regarding referrals to additional medical and treatment resources.
  • Schedule and coordinate MAT physicals as assigned, and participate in weekly case reviews and supervision.
  • Maintain accurate and up-to-date case records for each client.
  • Administer Vivitrol or Sublocade injections to clients diagnosed with Opioid Use Disorder (OUD).
