Position Overview:

Role Summary:

This position involves providing both clinical and technical assistance to primary care practitioners on a daily basis. The individual will execute all scheduled care tasks utilizing electronic systems to promote an efficient and precise workflow. Responsibilities include monitoring medical equipment and conducting clinical tests in compliance with OSHA and CLIA regulations. The role requires effective communication with patients, families, specialists, and all members of the healthcare team. In collaboration with the primary care nurse and physician, the individual will ensure the continuity and coordination of care within a patient-centered medical home framework. Support for the primary care nurse will include routine medication administration, immunizations, and other clinic-related tasks within the scope of LPN licensure.

Key Responsibilities:

  • Implement evidence-based guidelines from major regulatory and specialty organizations, and prepare for all primary care patient visits.
  • Collaborate with the primary care nurse and physician to conduct "e-huddles" using the Planned Care Dashboard, ensuring comprehensive primary care appointments.
  • Prepare patients for their appointments based on established criteria related to the nature of their visit (e.g., illness, physical examination, wellness check).
  • Document patient complaints and results from evidence-based screening tests accurately.
  • Measure and record patient vital signs with precision.
  • Independently conduct and document CLIA-waived tests, EKGs, and hearing & vision assessments.
  • Prioritize daily responsibilities to minimize wait times, working collaboratively with providers and support staff to facilitate patient flow.
  • Maintain a clean, organized, and safe work environment, ensuring the upkeep of supplies and medical logs as directed.
  • Prepare and stock examination rooms, ensuring cleanliness at the end of each workday.
  • Adhere to infection control protocols, follow Universal Precautions, and comply with safety policies and procedures of the organization. Maintain current CPR certification as required for clinical staff.
  • Exhibit exceptional customer service skills to both external and internal clients, focusing on quality care and teamwork that aligns with the organization’s mission.
  • Participate in mandated in-service training workshops as required.
  • Assist the primary care nurse with clinic support tasks, including routine medication administration and immunizations, under the appropriate delegation.

Qualifications:

  • Essential Skills and Education:
  • Completion of an LPN program.
  • Proficient in EKG operation, appointment scheduling, word processing, and computer skills, with familiarity in medical testing procedures.
  • Demonstrated organizational skills and attention to detail.
  • Excellent communication and customer service abilities.
  • Team-oriented with demonstrated leadership qualities.
  • Preferred Skills:
  • Bilingual capabilities.
  • Advanced knowledge of electronic health records.
  • Licenses/Certifications:
  • Current LPN license.
  • CPR certification from the American Red Cross or American Heart Association.

Physical Requirements/Work Environment:

  • Manual dexterity for clinical tasks, moderate physical activity including standing, walking, and lifting.
  • Potential exposure to bloodborne pathogens and biohazardous materials.
  • Frequent standing, walking, and sitting, with occasional bending, squatting, kneeling, and lifting up to 25 pounds.
  • Daily use of computers and telephones.
