As a key member of our team, you will be responsible for delivering high-quality patient care, working closely with our patients and their families to ensure the best possible outcomes.

Responsibilities
  • Provide one-to-one patient care in a home environment, executing physicians' prescribed plans of care and documenting all activities, assessments, and nursing actions in a timely manner.
  • Administer prescribed medication, treatments, and therapies, as well as perform patient assessments and coordinate care.
  • Participate in the development and update of the nursing care plan, taking into account the patient's unique needs and responses.
  • Recognize changes in patient needs and responses, implementing care to prevent risk or reduce risk, and accepting responsibility for personal and professional accountability.
  • Provide education to patients and their families regarding disease processes, self-care techniques, and prevention strategies, and in meeting the patient's nursing needs.
Requirements
  • Graduate of an accredited school of nursing.
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
Preferred Qualifications
  • Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
  • Home health experience.
Physical Requirements
  • Must be able to speak, write, read, and understand English.
  • Must be able to travel.
  • Must be able to lift 50 pounds.
  • Must be able to sufficiently reposition patients and move equipment without assistance.
