Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Home Care team. As a Primary Care Nurse, you will provide high-quality nursing care to patients in their homes, working collaboratively with our Population Health Case Managers to develop and implement individualized care plans.

Key Responsibilities
  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Conduct initial assessments and develop care plans in collaboration with Population Health Case Managers.
  • Supervise and support Home Healthcare Aides and LPNs as needed, ensuring seamless care delivery.
  • Implement therapeutic interventions, manage patient care, and achieve top decile outcomes.
  • Communicate effectively with patients, families, physicians, and other healthcare professionals to ensure comprehensive care.

Requirements
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum one year professional nursing experience.
  • Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit experience.
  • Strong computer and technology skills.
  • Current Driver's license and reliable transportation.
