Job Summary

We are seeking a highly skilled and compassionate Registered Nurse to join our Home Care team. As a Primary Care Nurse, you will provide high-quality nursing care to patients in their homes, working collaboratively with our interdisciplinary team to achieve optimal patient outcomes.

Key Responsibilities
  • Provide primary nursing care to patients in their homes, following physician orders and adhering to our organization's policies and procedures.
  • Develop and implement individualized care plans in collaboration with our Population Health Case Manager.
  • Supervise and support Home Healthcare Aides and LPNs as needed, ensuring seamless care delivery.
  • Utilize evidence-based practices and motivational interviewing techniques to achieve desired patient outcomes.
  • Manage patient care activities, including scheduling and coordination, to ensure efficient and effective care delivery.
  • Communicate effectively with patients, families, and healthcare teams to ensure comprehensive care and support.
Requirements
  • Graduate of an approved Nursing education program, BSN preferred.
  • Current Registered Nurse licensure in the state of practice.
  • Minimum of one (1) year professional nursing experience, preferably in Med-Surg, ER, Urgent Care, ICU, Critical Care, Progressive Care, or Step-Down Unit.
  • Strong computer and technology skills required.
  • Current Driver's license and reliable transportation.
