We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.
Key Responsibilities- Provide professional nursing services under the direction of a RN, adhering to state and national standards of care.
- Develop and implement individualized care plans to meet the unique needs of each patient.
- Conduct thorough assessments, identify patient needs, and develop effective care strategies.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Document patient progress, maintain accurate records, and communicate effectively with patients, families, and healthcare professionals.
- Graduate of an accredited LPN program or equivalent.
- Current LPN licensure in the practicing state.
- Valid driver's license, reliable transportation, and auto insurance.
- Current CPR certification.
- Minimum two years of experience as an LPN in a clinical setting, preferably in home health or hospice care.
