Essential Job Functions/Responsibilities
  • Patient Care:
    • Conduct a comprehensive assessment of patients and families to determine home care needs, providing a thorough physical assessment and history of current and previous illnesses.
    • Regularly re-evaluate patient nursing needs and make necessary revisions to the care plan.
    • Develop a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, involving the patient and family in the planning process.
    • Initiate appropriate preventive and rehabilitative nursing procedures, providing health care instructions to patients as needed.
    • Identify discharge planning needs as part of the care plan development and implement prior to discharge of the patient.
  • Communication:
    • Communicate with physicians regarding patient needs and report any changes in the patient's condition, obtaining/receiving physician's orders as required.
    • Communicate with community health-related persons to coordinate the care plan.
  • Additional Duties:
    • Participate in on-call duties as defined by the on-call policy, ensuring arrangements for equipment and other necessary items and services are available.
    • Instruct, supervise, and evaluate caregivers.
    Position Qualifications
    • Graduate of an accredited school of nursing with one to two years of recent acute care experience in an institutional setting.
    • Active nursing license in Maryland and CPR certification.
    • Bachelor's degree with one year of home health care experience preferred.
    • Management experience not required but recommended.
    • Excellent observation, verbal and written communication skills, problem-solving skills, basic math skills, and nursing skills per competency checklist.
    • Prolonged or considerable walking or standing, able to lift, position, or transfer patients, and able to lift supplies and equipment.
    • Considerable reaching, stooping, bending, kneeling, or crouching, visual acuity, and hearing to perform required nursing skills.
    • Licensed driver with an automobile that is in good working order and insured in accordance with the organization's requirements.
