We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a Full-Time RN Case Manager. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to patients and their families in the comfort of their own homes.
Responsibilities- Conduct comprehensive assessments of patients' physical, functional, psychological, and environmental needs.
- Develop and implement individualized plans of care to ensure quality and continuity of care.
- Collaborate with patients, families, and healthcare professionals to provide education and support.
- Monitor patient progress and make adjustments to care plans as needed.
- Provide clinical direction to Hospice Aides and LPNs/LVNs to ensure quality and continuity of service.
- Document patient information and progress in a timely and accurate manner.
- Graduate of an accredited Diploma, Association, or Baccalaureate School of Nursing.
- Current license as a Registered Nurse (RN) and/or accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- Two years' experience as a Registered Nurse in a clinical care setting, with hospice experience preferred.
- Nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills and ability to work effectively with patients, families, and healthcare professionals.
